Cred că SQL Server tb. să se ocupe de date şi nu de download-at email-uri.
Poţi realiza o aplicatie simplă în C# / VB.Net care să
[1] citească email-urile de pe un server,
[2] apoi să analizeze textul din subiect sau textul din conţinut
[3] şi dacă găseşte (în subiect sau în conţinut) anumite cuvinte cheie
[4] deschide conexiunea la un server SQL şi execută procedura stocată.
